Applications

The forms available as Adobe Acrobat® (.PDF) files are interactive format (except renewal forms and new form CE6). Users can complete information fields while viewing the form on the website or download a blank copy of the form to their own computer. Users must have:

  1. Adobe Reader 6® or higher installed on their computer.
  2. Web browsers (Internet Explorer, Mozilla, etc.) set to allow viewing of Adobe/PDF files.

TO COMPLETE FORMS ONLINE:
Select a PDF document, type in information required in the respective fields and then print.  IF YOU ARE HAVING TROUBLE OR THE FORMS ARE NOT PRINTING THE INFORMATION YOU ENTERED, PLEASE CHECK TO MAKE CERTAIN YOU HAVE THE REQUIRED VERSION OF ADOBE READER®.

TO DOWNLOAD AND SAVE A BLANK COPY OF A FORM:
Select a PDF document from this page. Once opened in your web browser, go to the "File" menu on your toolbar. Select "Save As" and a window will open to allow users to save the document. Users can save the document as named or change the name. Once saved, users can simply access the document from their own computer without going to the website. IF YOU ARE HAVING TROUBLE PLEASE CHECK TO MAKE CERTAIN YOU HAVE THE REQUIRED VERSION OF ADOBE READER®.

THIS FEATURE DOES NOT ALLOW USERS TO SAVE COMPLETED ELECTRONIC VERSIONS:
Users will not be able to save a completed electronic copy unless they have their own forms management software program installed.  Users may save a blank copy of the form to their own computer files for more convenient access later.

PRINTING:
When a user selects the print button, a printing window will open. Please make certain the paper size displayed in the printing window matches the proper document size. Adobe uses 81/2 X 11 (letter sized) as the default paper size however pre-need contracts as well as pre-need statements of goods and services are on 81/2 X 14 (legal sized) paper. Users may have to change the paper size in the printing program. In the printing window, click on the "PROPERTIES " button at the top right. That will open another window with two tabs "Layout" and "Paper/Quality" Either of these tabs will display an "ADVANCED"  button on the bottom right. Click on this and another window will open. Under the area "Paper/Output" there is a drop down menu. Click on the drop down arrow to the right of that menu and you will see a selection of document sizes. Choose the appropriate size for the document you are printing. 

You may print the PDF. version and fill in by hand or typewriter. Click on the link for the format you want.  Please notice the re-arrangement of the placement for each format. The Word documents have been moved to the right side column with the Adobe PDF documents now in the center column. Some documents are on legal size paper (8 1/2 x 14) which is noted beside by the document name.
