On-line Payment

  • Effective September 2017, applicants and licensees may pay fees to the Board by credit card with the following exceptions:
  •  
    • Compromise penalties owed pursuant to the terms of a consent order or final agency decision must be paid to the Board by check.
    •  
    • Preneed filing fees cannot be paid by credit card at this time.
    •  
    • Application fees are non-refundable, regardless of whether a submitted application is approved or not. Please note that online payment of application fees does not guarantee that your application will be approved.

  • Instructions and Guidelines
  •  
    • Provide a brief explanation of the fee being paid in the “Description” box.
    •  
    • Put the applicable permit or license number for which the fee is being paid in the “Customer ID” box. New applicants for licensure should put "None" in "Customer ID" box.
    •  
    • Debit card transactions will be processed as a credit transaction. All activity should be reflected in your account within 2-4 business days.
