To order a duplicate wall certificate for an individual license (embalmer, funeral director, or funeral service), please click on the green button below. Follow the prompts and complete the payment form for $25.00 per license certificate. In the Description box: (1) indicate how many certificates you wish to order and for whom; (2) include the license number(s) for each duplicate; (3) provide the shipping address.
If you need a replacement business license (at-need or preneed funeral establishment), please email your request to: permitapplications@ncbfs.org. There is no fee for these certificates.
If you need to order a replacement pocket card for an individual license, please call the Board office.
Upon completion of your online payment, the Board will receive an authorization with the information you provided. The Board will then order your certificate(s). License certificates are produced and delivered every 3-4 weeks.
